How to create a Report

Creating a report will provide a snapshot of your symptoms, medications, health data and treatment regime. Before your next appointment, create a report to help you feel empowered to advocate for yourself.

 

Create a report

  1. Select "reports" located under “my documents”
  2. Click on “create a report”
  3. Enter a title for your report
  4. Select a suggested timeline or create a custom timeline
  5. Select previously entered symptoms, questions, medications/supplements, images/documents, and graphs will automatically populate
  6. Click on “generate report” located at the bottom of the screen

Once the report has been generated, you can download it, by clicking the "download" option located at the bottom of the screen.

Once downloaded you can bring a copy to your appointment, or email a copy to your care team.
